Registered Nurse (RN)

The nurse is an essential care team member in The Green House Model of long term care. The Green House Model is about building a human habitat where Elders and staff are living rich and meaningful lives. At the same time, Elders being served in The Green House homes have complex skilled nursing needs. Nurses need to assess and meet those clinical needs while supporting the quality of life of each Elder. Nurses have multiple roles to fulfill to be effective in meeting the needs of Elders and staff in The Green House Home.

The Green House model is grounded in the notion that Elders have the right to excellent, evidence-based medical and clinical care.

Quality of care is critical. But quality of life needs to be first in consideration of all care and treatment. The nurse in the Green House home has the opportunity to practice in expanded professional roles to achieve person-directed care in a place called home.

In addition to their roles in providing clinical assessment, treatment, and evaluation for the Elders, nurses are also in partnership with the CNA’s and bring the following four professional roles to their relationships with CNA’s:

Certified Nurse Assistant (CNA)

In the Green House Model of Long Term Care, the Certified Nursing Assistant is also known as a Shahbaz. The Shahbaz will care for the elders living in a Green House Home. Their primary role is to protect, sustain, and nurture the Elders by providing assistance with activities of daily living and meeting other needs as required. The core goal of the Shahbazim’s work is to provide assistance in a manner that promotes a high quality of life and a positive Elderhood. The responsibilities of the Shahbazim include care of the Elders and the Elders’ environment, including cooking, laundry, and housekeeping.

The Shahbaz is a member of a self-managed work team and is responsible for coordinating aspects of the team’s work.

Responsiblities:

  • Recognize and respond to the needs of Elders
  • Promote autonomy of Elders in decision-making
  • Maintain confidentiality and privacy of Elder care, procedures, and documentation
  • Communicate with co-workers at all levels to meet the needs of Elders
  • Meet with the CST as scheduled or needed to discuss Elders’ needs and progress in identifying and correcting problems
  • Observe, monitor, and report to the CST any change in condition
  • Document observed data per organization policy
  • Assist Elders with personal care functions including:
    • Dental and Mouth Care
    • Bathing
    • Dressing and Grooming
    • Hair Care
    • Nail Care
    • Shaving
    • Bowel and Bladder Care
    • Other Personal Hygiene Care
  • Assist with and/or perform restorative and rehabilitative procedures as outlined and assigned by the care plan and with the appropriate training
  • Measure and record vital signs such as temperature, pulse, respirations, weight, height, and anatomical dimension/circumference
  • Assist moving, positioning and transporting Elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Maintain a manual-lift free environment by utilizing the lift systems provided in the houses
  • Assist with the application of slings, elastic bandages, binders, etc.
  • Provide range of motion exercises as outlined and assigned by the care plan and with appropriate training
  • Prepare and serve meals, between meal, bedtime and other nutritional snacks per posted menu and the stated preferences of the Elders:
  • Observe standard precautions per organizational policy
  • Keep all areas of the home clean, performing routine housekeeping duties
  • Make occupied and unoccupied beds and change bed linens
  • Launder clothing, linen, and other articles
  • Follow established safety precautions and observe, monitor, and intervene in or report to the Guide and unsafe conditions
  • Assure Elder calls for assistance are answered promptly
  • Report accidents and incidents when they occur
  • Report any abuse or suspected abuse to the Guide immediately
  • Follow established smoking regulations and report violations
  • Attend and participate in orientation programs, on-going training, and educational classes
  • Perform other related duties as required by the Elder

Accountability

The Shahbaz works cooperatively with other Shahbazim on the self-managed work team. The team and each individual Shahbaz works collaboratively with the CST. The CST directs all matters related to the skilled nursing services. The self-managed Shahbazim work team and individual Shahbazim report to the Green House Guide. The Shahbazim seek counsel and assistance with problem solving from the Sage.

MDS Coordinator

The Green House Cottages of Poplar Grove is seeking a LPN MDS Coordinator to work with our team at Central Arkansas’ premier long-term care and rehabilitation facility!

The MDS Coordinator participates in the Resident Assessment Process through completion of the pertinent items on the Minimum Data Set (MDS) and the accompanying Care Area Assessments (CAAs).The MDS Coordinator also directs the Resident Assessment Protocol Summaries (RAPS). Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes; as required by Federal and State regulations. He/she will monitor nursing services to ensure that resident’s needs and rights are met and promptly reports to the Administrator and maintains written records of all resident complaints and grievances.

RESPONSIBILITIES:

1. MDS/RAI Care Plan Functions. Direct the facility interdisciplinary RAI process while following HHC guidelines for completion, computer encoding and electronic State transmission of the MDS.

2. Complete nursing assigned MDS items, RAPS and Care Plans asdesignated by HHC.

3. Participate in the resident Care Plan Meetings.

4. Train new members of the IDT on clinical content and electronic completion of the applicable MDS items,

5. RAPS and writing of individualized resident Care Plans.

6. Participate in the daily Morning Meeting per HHC guidelines.

7. Print Nursing Home Quality Indicator Reports from the State data base every month and as requested by the facility Director of Nursing.

8. Highlight Quality Indicators that are above the 75th percentile, the 90th percentile and triggered Sentinal Events, and distribute to the facility Director of Nursing and Administrator.

9. Maintain resident confidentiality and privacy; treat residents with kindness, dignity and respect; know and comply with and ensure that all nursing services personnel know and comply with Resident’s Rights rules

10. Physical and Sensory Requirements (With or Without the Aid of Mechanical Devices): Ability to communicate with residents and personnel and ability to remain calm under stress.

11. Maintains on-site regular attendance as scheduled.

12. Complies with applicable legal requirements, standards, policies and procedures including but not limited those within the Compliance Process, Code of Conduct and HIPAA.

13. Provides leadership and support for the Compliance Process within management area.

14. Promotes adherence to applicable legal requirements, standards, policies and procedures as specified within the Compliance Process, Code of Conduct, Federal False Claims Act and HIPAA within management area.

15. Distributes compliance-related materials within management area.

16. Ensures timely and accurate reporting and responses to compliance-related issues and monitors the implementation of corrective action plans related to such issues.

17. Ensures that staff participates in orientation and training programs including but not limited to all required compliance courses and relevant policies and procedures, and that such training is properly documented.

18. Provides open lines of communication regarding compliance issues within management area through the Four Step Reporting Process and access to the confidential disclosure program reporting lines and ensures that retaliation against staff who report suspected incidences of non-compliance does not occur. Reports concerns and suspected incidences of non- compliance using the 4-Step Reporting Process.

19. Participates in monitoring and auditing activities and investigations, and implementing quality improvement processes, as required.

20. Participates in compliance and other required training programs.

COMPLIANCE

Complies with applicable legal requirements, standards, policies and procedures including but not limited to those within the Compliance and Ethics Program, Standard/Code of Conduct, Federal False Claims Act and HIPAA.

QUALIFICATIONS:

  • Must be an LPN in good standing and currently licensed by the State
  • Must show ability to read and write in a legible and understandable manner
  • Previous experience in a hospital, long-term facility, or other healthcare related facility
  • Knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures and laws, regulations and guidelines governing long-term care
  • Positive attitude toward the elderly
  • Previous experience as an MDS Coordinator is preferred
  • Previous experience in completing MDS’s and Care Plans is required.
  • Maintains current BLS/CPR certificate (excluding ALFs/ILFs)
