Registered Nurse (RN)

The nurse is an essential care team member in The Green House Model of long term care. The Green House Model is about building a human habitat where Elders and staff are living rich and meaningful lives. At the same time, Elders being served in The Green House homes have complex skilled nursing needs. Nurses need to assess and meet those clinical needs while supporting the quality of life of each Elder. Nurses have multiple roles to fulfill to be effective in meeting the needs of Elders and staff in The Green House Home.

The Green House model is grounded in the notion that Elders have the right to excellent, evidence-based medical and clinical care.

Quality of care is critical. But quality of life needs to be first in consideration of all care and treatment. The nurse in the Green House home has the opportunity to practice in expanded professional roles to achieve person-directed care in a place called home.

In addition to their roles in providing clinical assessment, treatment, and evaluation for the Elders, nurses are also in partnership with the CNA’s and bring the following four professional roles to their relationships with CNA’s:

  • Care Role Models
  • Gerontological Nurses
  • Care Team Partners

Certified Nurse Assistant (CNA)

In the Green House Model of Long Term Care, the Certified Nursing Assistant is also known as a Shahbaz. The Shahbaz will care for the elders living in a Green House Home. Their primary role is to protect, sustain, and nurture the Elders by providing assistance with activities of daily living and meeting other needs as required. The core goal of the Shahbazim’s work is to provide assistance in a manner that promotes a high quality of life and a positive Elderhood. The responsibilities of the Shahbazim include care of the Elders and the Elders’ environment, including cooking, laundry, and housekeeping.

The Shahbaz is a member of a self-managed work team and is responsible for coordinating aspects of the team’s work.

Responsiblities:

  • Recognize and respond to the needs of Elders
  • Promote autonomy of Elders in decision-making
  • Maintain confidentiality and privacy of Elder care, procedures, and documentation
  • Communicate with co-workers at all levels to meet the needs of Elders
  • Meet with the CST as scheduled or needed to discuss Elders’ needs and progress in identifying and correcting problems
  • Observe, monitor, and report to the CST any change in condition
  • Document observed data per organization policy
  • Assist Elders with personal care functions including:
    • Dental and Mouth Care
    • Bathing
    • Dressing and Grooming
    • Hair Care
    • Nail Care
    • Shaving
    • Bowel and Bladder Care
    • Other Personal Hygiene Care
  • Assist with and/or perform restorative and rehabilitative procedures as outlined and assigned by the care plan and with the appropriate training
  • Measure and record vital signs such as temperature, pulse, respirations, weight, height, and anatomical dimension/circumference
  • Assist moving, positioning and transporting Elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Maintain a manual-lift free environment by utilizing the lift systems provided in the houses
  • Assist with the application of slings, elastic bandages, binders, etc.
  • Provide range of motion exercises as outlined and assigned by the care plan and with appropriate training
  • Prepare and serve meals, between meal, bedtime and other nutritional snacks per posted menu and the stated preferences of the Elders:
  • Observe standard precautions per organizational policy
  • Keep all areas of the home clean, performing routine housekeeping duties
  • Make occupied and unoccupied beds and change bed linens
  • Launder clothing, linen, and other articles
  • Follow established safety precautions and observe, monitor, and intervene in or report to the Guide and unsafe conditions
  • Assure Elder calls for assistance are answered promptly
  • Report accidents and incidents when they occur
  • Report any abuse or suspected abuse to the Guide immediately
  • Follow established smoking regulations and report violations
  • Attend and participate in orientation programs, on-going training, and educational classes
  • Perform other related duties as required by the Elder

Accountability

The Shahbaz works cooperatively with other Shahbazim on the self-managed work team. The team and each individual Shahbaz works collaboratively with the CST. The CST directs all matters related to the skilled nursing services. The self-managed Shahbazim work team and individual Shahbazim report to the Green House Guide. The Shahbazim seek counsel and assistance with problem solving from the Sage.

Duties and Responsibilities

  • Provide leadership and guidance to ensure that food quality, safety standards and resident satisfaction are properly adhered to and maintained.
  • Assure safe receipt, storage and preparation of food, hire and manage food service employees, prepare and maintain work schedules, motivate staff and maintain excellent employee relations, provide nutrition consulting, participate in menu planning, control inventory, maintain food costs within budget and perform food and supplies purchasing
  • Prepare cleaning schedules and maintain equipment to ensure food safety, work cooperatively as part of an interdisciplinary team and maintain positive working relationships with Green House Team and other service providers
  • Care planning and MDS Documentation
  • Maintain compliance with State/Federal Regulation

Requirements

  • Must be a licensed Certified Dietary Manager. Designation is required in accordance with CMS and/or State/County law
  • Associate’s degree or higher in food service management from an accredited institution of higher learning
  • A love of food service and logistics!
  • Specialized training in foodservice management and nutrition
  • Current ServSafe certification is required in accordance with State/County law
  • Certified Dietary Manager designation is required in accordance with CMS and/or State/County law
  • One year or more in dining/nutrition supervisory experience
  • Ability to read and understand nutrient analysis reports
  • Knowledge of foodservice program requirements
